I want to get a criminal record certificate. What should I do?

A.

Applicants Eligible for the Certificate:
The Hyogo Prefectural Police (HPP) Headquarters (HQ) Identification Division will issue a criminal record certificate to individuals who are required to submit it to foreign authorities (an embassy/a consulate) and to individuals:
  • who are currently registered residents (foreign residents) in Hyogo.
  • whose last resident registration (foreign registration) in Japan was in Hyogo.
  1. Reception Hours:
    Monday to Friday 9:00-11:30, 13:00-17:00
    (Not available on Sat., Sun., National Holidays, and year-end/beginning Holidays.)

Application Procedures
  1. Telephone Inquiries
    Please make inquiries by phone in advance, as there may be cases where your application cannot be accepted immediately (if it does not meet the issuance requirements) or where the certificate is not necessary, depending on the submission destination and the reason.
  2. Application
    1. Please come to the reception on the first floor at HPP HQ during the above time and inform the reception staff of your requirements. No appointment is necessary.
    2. The applicant must come in person. (Proxy applications are absolutely not allowed due to the need for the applicant’s fingerprints.)
    3. The Identification Div. will handle the application reception.
    4. It takes two weeks from the date of application to issue the criminal record certificate.
    ● Required Documents for Applicants living in Japan
    1. ① Passport (valid)
    2. ② One of the following (official documentation verifying your address in Hyogo):
      1. Residence Card, Special Permanent Resident Certificate
      2. Copy of the Resident Record (issued within the last six months)
      3. Driver’s License (valid, with address and name matching those reported to the authorities)
      4. Individual Number Card (My Number Card)
    3. ③ Documents recognized as requiring the submission of a criminal record certificate Documents requesting the submission of a criminal record certificate, created by the submitting institution (such as an embassy or other official institution), that include the applicant’s full name. (e.g., Issuance Request Letter, etc.)
    ● Required Documents for Applicants living outside Japan
    1. ① Passport (valid)
    2. ② Copy of the Deleted Resident Record/the Supplementary Family Register (issued within the last six months)
    3. ③ Documents recognized as requiring the submission of a criminal record certificate Documents requesting the submission of a criminal record certificate, created by the submitting institution (such as an embassy or other official institution), that include the applicant’s full name. (e.g., Issuance Request Letter, etc.)
  3. Issuance of the Criminal Record Certificate
    1. The receiving location of the criminal record certificate is the same as the application location. The applicants must come in person. However, if there are unavoidable reasons, an authorized representative may receive it on behalf of the applicant with a letter of attorney. (Not allowed by post.)
